BENEFIT STARTS SEPTEMBER 1st, 2009
The Board of Trustees of Local 1201 Health and Welfare Fund (the Fund) is proud to announce a NEW benefit program - HEARTCAM Preventive Heart Health Screenings. This new program will benefit the overall health of all participants. Because heart disease is consistently one of the largest health risks facing our participants, the Fund is offering this safe, easy, non-invasive screening to detect disease at its earliest phase, before a potentially life-threatening heart attack can occur.
HeartCam is a service provided in collaboration with University of Pennsylvania, Presbyterian Medical Center at 38th & Market Streets in Philadelphia (free parking is available in the Presbyterian Garage). This heart scan, for the early detection of disease will be offered as a fully paid benefit to any eligible participant, a $400 value! There is NO CO-PAY! (Spouses / dependents are not covered under this benefit.)
HeartCams heart scanning detects any plaque in your coronary arteries and accurately predicts your risk for heart attack before you experience any symptoms and well before a stress test. This is important because stress testing misses most early heart disease and many heart attacks occur in patients who have already passed a stress test. And, HeartCam takes just minutes. There is no preparation or dyes. With early detection, coronary plaque can be slowed, stopped or possibly reversed.
HeartCam Eligibility Requirements:
You are eligible if you are over 45 years old with no prior history of heart disease and are not pregnant, and have THREE or MORE of the following risk factors:
Family history of heart problems
Over-weight (note, if you weigh over 300 lbs.,
you may be accommodated at another comparable heart scanner at Presbyterian)
High cholesterol
High blood pressure
Smoker or ex-smoker
Diabetes
If your waist size is more than 40 inches (men) or 35 inches (women) you may be eligible for a free metabolic clinical research health screening. Please inquire when scheduling.
In addition, you must obtain a prescription from your doctor for the screening. You can either bring the prescription with you at the time of your appointment or ask your doctor to fax the prescription directly to HeartCam. If you do not have a doctor, you should contact HeartCam and the HeartCam Medical Director will provide you with a prescription for the screening, provided you are otherwise eligible for the screening. This fully paid benefit is available to eligible participants beginning September 1st, 2009.
To schedule your screening, you must first call the Fund Administrator at 215-627-9220 to receive a confidential history questionnaire. Once you have completed the questionnaire, you must send it by mail or fax to HeartCam for eligibility verification. STAY HEALTHY!
